Medication Management
Our staff will provide reminders with medication in-take to ensure the residents’ compliance with their medication therapy plan. In addition to reminders, we also handle the following medication related services.

- Coordinating with pharmacy for refills and re-orders
Personal Care

Frequently Asked Questions
When a request for home care services is received, an evaluation of the needs of the client will be made and if we can provide a sufficient qualified staff to meet those needs. A final decision will be made by Registered Nurse and Administrator at the time of the assessment.
We have an identified plan in place to assure the safety and well-being of clients and staff during periods of an emergency or disaster that disrupts services
Resident will only be admitted into the medication management program at the sole consideration of the staff Registered Nurse after an in-person assessment has been performed
Confidentiality is an essential ingredient in gaining and maintain client trust. We respects that an integral part of providing quality care is to gain the trust of the client to work on mutually agreed upon goals.
We provide and individualized service plan implemented for all residents and provide services required by the current service plan
